How to use the Yellow Card

The purpose of the Yellow Card is to report SUSPECTED Adverse Drug Reactions and Adverse Drug Events.   There is a great concern that it is very much under used, by both professionals and the public.  With concerns being raised regarding medicine safety from different  disciplines there is a clear need to take a more pro-active role in our own health care.   We urge you to use and promote the yellow card to this end.

Your can also report:

  • Defective Medicines (that are not of acceptable quality)

  • Counterfeit Healthcare Products  

  • Blood factors and immunoglobulin products

  • E-cigarette products

Did you know? 

  • You can use your Patient Information Leaflet, that came in the box of your medicines to help.

  • It is important to be thorough, and to include as much as you can.

  • It is useful to report if you get a lot of colds - this shows your immune responses. 

  • If you take medicines regularly then you should receive a medicine review every year.  

  • If you take long term medication you can ask for a review with your pharmacist every year.
